When you have decided which gift card(s) and amounts you want to receive, please:

1) Enter the # (QUANTITY) of each gift card you select (NOT the dollar amount)

2) The form automatically calculates the grand total dollar amount for you at the bottom (= # quantity * gift card $ amount)

3) **Please verify that the 'Grand Total Amount' (at the bottom) matches the amount you received as a gift (found in your email).**
