You can edit the Title option in the Theme settings for your account to update the account name / header displayed in your forms. Login to Formfacade. Click on the form to open it. Edit page will be displayed. In the Edit page, click on the profile name in the top right corner. Click Change theme. Theme settings will be displayed in the Preview page. Click Page. Enter the required text in the Title (header / account name), Footer option (replaces "This site belongs to...") and click Save. Please note that the theme settings is for your account and it will be applied to all the forms you create using your account.
