Hi Judy, you customize the header, footer and theme for your account.

Login to Formfacade. Click on the form to open it. Edit page will be displayed. In the Formfacade Edit page, click on the profile name in the top right corner. Click Change theme. Theme settings page will be displayed in the Preview page. Click Page. Enter the required text in the Title (header / account name), Footer option (replaces "This site belongs to...") and click Save. You can upload your logo to replace the default profile picture.
