When you sync the Google Forms responses to Google Sheets, the question title will be added as the column header and each row will be a response. The description will not be included in Google Sheets.
If you would like to collect signatures in a consent form, then you can use Formesign addon to setup your form and sync the signed document to Google Drive. For detailed instructions, refer to the help article below.
