                    Hi, I am trying to set up a form for customer to upload pictures to prove that the corals we sold are death on arrivals. I am hoping to make a section where customer can put the name of the died coral, and then upload a picture next to the response. If you have already created your form with a file upload question in Google Forms, then you can customize it using the Petaform addon to convert the Google Forms file upload that requires login into html file upload that allows users to upload files without login. Open your form in Google Forms > click on the addon icon > click Petaform > click Configure form > click Yes to convert the Google Drive file upload into standard html file upload question > click Proceed > Edit page will open in a new tab > You can click on the gear icon next to the file upload question to set the max files allowed for this question and specify the allowed file types > click Share > copy the short link and share it with your users to collect responses. You can also set up sync so that the uploaded files are automatically synced to your Google Drive folder when the user submits the form.
