You can add a multiple choice or dropdown question with different categories as responses. For example, Never, Rarely, Sometimes, Always. If you have already set up your questions in Google Forms, you can use the Scorecery addon to assign points and add score calculations in your google forms OR use the Scorecery templates to create your form. Assign points, category In the Scorecery Edit page, click on the gear icon next to the question (in the new editor, you will have to click on a question to select it and then click on the gear icon for that question) > Field settings page will be displayed > click Answer > assign the points for different answer choices > click Save. Calculate score (If it is not automatically added in the form) In the Scorecery Edit page, click on the + icon to add a short answer question > enter the title "Total score" > click on the gear icon for this question > Field settings page will be displayed > click Answer > enter the formula POINTS() in the Calculate option and click Save. You can also calculate section scores or assign different categories to questions and calculate category scores.
