I cannot log into my survey. I desperately need to. All my study data is in your system. Please help me. I contacted you earlier and it was not helpful. I need to know the email address I used to create the form.

Hi Suzanne, we cannot share the email used to create the form you have shared unless you can provide information that allows us to verify that you have created the form.
Note: Formfacade use Google login for signin. You can login to google forms or Formfacade using your different google accounts and check if this is google form is listed in any of these accounts.
