Hi Nirmal, We do not ask for card details when you install the addon/create your account. If you find Formfacade useful and would like to continue using it after the trial ends, you can subscribe to the paid plan. There is no payment linked with your account. The charges you are seeing on your bank statement is not from Formfacade.

If you have installed the addon and would like to uninstall/remove the app from your Google Forms, here's the instructions: Open your Google Forms > click on the three dots icon next to the SEND button > click Addons > search for Formfacade > select Formfacade > click on Uninstall. If you would like to delete your account, please confirm.
