Hi Silvia, You should make a copy of the form in Formfacade to duplicate it with the customization.

Login to Formfacade > click Forms > list of your forms will be displayed > click on the More option for the form you would like to duplicate > click Make a copy > enter the form name and click Proceed > Edit page will be displayed > You can make any required edits and use the share link to collect responses.
