Folders
When you select a folder in your drive, the addon will automatically create a folder for each file upload question in your form. The uploaded files will be synced to the relevant folders.
File name
When you set up the sync to drive, we can also configure the form to include the name or email field in the file name. The default file name is PF#####_filename where PF##### is the unique response id generated by Formfacade and the filename is the name of the file as uploaded by the user.
Map customer fields
Open your form in Google Forms > click on the addon icon > click Petaform > click More options > click Customize > Edit page will open in a new tab > In the Edit page, click on the gear icon next to the Form title > Form settings page will be displayed > click on the configure fields icon next to the Save button in the bottom right corner > Respondent settings page will be displayed > select the relevant fields for the Name, Email option and click Save. When the email field is mapped, it will be included in the file name. For example, PF#####_<email>_filename. Otherwise, if the name is mapped, the file name will be PF#####_<name>_filename

Hi Riaz, you cannot create a separate folder for each response. When you setup the sync to drive option using the Petaform addon, you can select a folder in your Google Drive. The addon will automatically create subfolders, one for each file upload question. You can only rename the files to include the name or email based on a form field.
