Hi - You have added an email field in Google Forms, but this change has not been synced with Formfacade. When the user fills the form and submits it, the response for this required email field is missing. Google Forms will reject responses with missing answers for required fields. That's why you are getting the submit failed error. Open your form in Google Forms > click on the addon icon > click Formfacade > click More options > click Update or Customize to sync the changes with Formfacade immediately > check if the email field is displayed in the first page in the form and submit a response to test the form. Please try this and confirm.
