Hi Mrudhuna, Once you customize your form using Formfacade, you will have to use the near.tl link for your form to collect responses. Otherwise, the customization and file upload will not work. Login to Formfacade > click Forms > click on the form to open it > Edit page will be displayed > click Share > In the Share page, click Link > copy the short link and use it to collect responses.
