Formfacade account Formfacade uses Google signin. We do not have the option to create a username or an account by entering an email and password. If you would like to use a different email account, then you can subscribe to the paid plan using the preferred email account and cancel the existing subscription. Customize header, footer, theme Login to Formfacade. Click on the form to open it. Edit page will be displayed. In the Formfacade Edit page, click on the profile name in the top right corner. Click Change theme. Theme settings page will be displayed in the Preview page. Click Page. Enter the required text in the Title (header / account name), Footer option (replaces "This site belongs to...") and click Save. You can upload your logo to replace the default profile picture. The theme settings are for your account. All the forms you create using your account will have the same theme, header, footer.
