Hi - The subscription is linked with the email account used to subscribe to the paid plan. Only that account can be used to create the form and collect responses. You can subscribe to the paid plan using the preferred email account and use the embed code from that account to add the form on your website. Note: The forms were created in Google Forms and customized using the Formfacade addon. Google Forms does not allow you transfer ownership across domains. You can ask them to add your email as a collaborator in Google Forms to get access. You can either choose to use the same form or make a copy of the form, customize it using the addon and use the embed from your account.
