Hi - If you have logged in with multiple google accounts in your browser, sometimes Google Workspace addons may not be installed properly. Open an incognito window, login with the google account you would like to use, install the addon from Google Workspace marketplace and then use the Formfacade addon. It should work. Please try this and confirm.
