Hi Lauren, the subscription linked with Michelle's email account was cancelled in May and the annual subscription expired on 3-June. When you login to Neartail using Michelle's account, it will show an upgrade message.

HI Lauren, You should write to your domain administrator and request to them to update permissions / add the Formfacade addon to the allowlist.

Note: Michelle was using Neartail for creating the uniform order form. Your administrator would have already added Neartail addon to the allowlist. Please try it.
