Login to Neartail. Click on the form to open it. Edit page will be displayed. In the Neartail Edit page, click on the profile name in the top right corner. Click Change theme. Theme settings will be displayed in the Preview page. Click Page. Enter the required text in the Title (header), Footer option (replaces "This site belongs to..."), upload your logo to change the default profile picture and click Save. Please try this and confirm.
