Login to Neartail > click on the form to open it > Edit page will be displayed > In the Neartail Edit page, click on the form title to select it > click on the Advanced settings gear icon > Form settings page will be displayed > click on the Rearrange icon (see screenshot below) > Rearrange settings page will be displayed > you can drag and drop fields to arrange it in the preferred order.
