I need to create One Form for both Delivey/Pickup. Need to have one select button where customer can select either Deilivery or Pickup. Based on customer selection, we need to present Delivery or pickup form to the customer. Please help
You can add a dropdown or multiple choice question with different pickup and delivery options as answer choices. Mark this as a required question so that users have to select an answer to submit the form.
Add delivery, pickup options
Login to Neartail > click on the form to open it > Edit page will be displayed > In the Edit page, click on the + Add field icon (new editor) > enter the title "Delivery or Pickup”, select Dropdown or multiple choice question type > enter the different pickup and delivery options as the answer choices (see below) and click Save.

Delivery to Y, Z
Delivery to X
Currently we do not have the option to show / hide questions based on answers. You can add a paragraph question to collect delivery address. Leave this question as optional so that users who select delivery can enter their complete delivery address.
Add delivery address
Login to Neartail > click on the form to open it > Edit page will be displayed > In the Edit page, click on the + Add field icon (new editor) > enter the title "Delivery address", select Paragraph question type.
