Yes, you can create a duplicate of your form and make the necessary changes for a different use case. Login to Neartail > List of your forms will be displayed in the Forms dashboard > click on the "More" option for the form you want to duplicate > click Make a copy > enter the form title and click Proceed > Edit page will be displayed > make the necessary changes and click Share > copy the short link and use it to collect responses.
