Hi Teresa, when you subscribe to the paid plan, you can either choose the monthly or annual option. Monthly subscription is automatically renewed each month. You can cancel it anytime. Annual subscription is a 12 month contract. If you need the form only for a short term of 3 months, then you can subscribe to the monthly option end of May or in June, use it for 3 months till Aug and then cancel the subscription before it is renewed for the 4th month. We use Stripe for subscription payments and it is set up to automatically send a reminder email 3 days before the subscription is renewed so that users can choose to continue or cancel the subscription.

So the Form building would be the $16/month. For me to completely build the form with the payment option, I would need to start paying now? Is there an additional cost for the Google Forms payment addon for the google form? Is that a one time or a per/month.
