Hi Elizabeth, You have closed your from in Google Forms. That's why you are seeing the message "The form Gradventure Celebration June 4 is no longer accepting responses".
Open your form in Google Forms > click Responses > enable Accepting responses option > click on the addon icon > click Neartail / Peergateway addon > click More options > click Update to sync the changes immediately. Once you make this change, you can preview the form and test it.
