Hi - If you have logged in with multiple google accounts in your browser, sometimes Google Workspace addons may not be installed properly. Open an incognito window, login with the google account you would like to use and the install the Peergateway addon from Google Workspace marketplace. When you click on the addon icon in Google Forms, the installed addons will be displayed. You can click on the Peergateway addon to calculate the amount and enable payments in your form. Please try this and confirm.
