1. Who fills the form -- Client or the Sales person?
2. How do they access the form? Have you embedded the form on your website or added the Formfacade share link as a hyperlink or a button or do you email the link to them?
3. What's your use case? How do you plan to use the information about the sales person?
1. The customer fills the form and is allowed to on send it.
2. They recieve the form either by email or whatsapp. It will be embedded in the website later
3. This is to track the sales attributable to them to enable get their commission.
You can add a short answer question "Referral code" in the form and ask the clients to enter the relevant code for the sales person when they submit the form. You can use this field to track and attribute sales to different sales person.
Instead of asking the user to enter the referral code, you can create unique prefill links for each sales person. Login to Neartail > click on the form to open it > Edit page will be displayed > In the Neartail Edit page, click on the + icon to add a short answer question > enter the title "Referral Code" > click on the gear icon for this question > Question settings page will be displayed > click Answer > change appearance to hidden or read only and click Save.

in to Neartail > click on the form to open it > Edit page will be displayed > click Share > In the Share page, click Customer > click Links > click New link > New link setup page will be displayed > enter the unique code, click Next > enter the answer for the referral code question and click Create > repeat the steps to create links for each sales person in your team.
