My form is once again not accepting responses and now when linked to sheets there is a new column labeled score. I am not sure what data field it applies to.

Hi Elizabeth, If you enable the quiz option in Google Forms, then when you link your Google Forms with Google Sheets, it will automatically add a column "Score" after the timestamp column. Open your form in Google Forms > click Settings > In the Settings page, disable Make this a quiz option > click Responses > In the Responses page, click on the three dots more icon > click Unlink form > you can then link the form with Google Sheets. The linked Google Sheet will not have the score column. Please try this and confirm.

When I link the sheet again will I lose the previous responses?

If you haven't deleted the responses from Google Forms, you will not lose any data. All the responses in Google Forms will be synced to the new google sheet. Note: [1]. When you unlink the form, the existing Google Sheets will not be deleted; we are only unlinking the form. You can always access this sheet from your Google Drive. [2] When you link the form again, you can create a new google sheets or choose to select existing google sheets. If you select the existing sheet, it will create a new tab in the sheet and sync the responses.
