Hi, I do have ExtendedForms but I did not add it to this form. should I uninstall?

Hi Elizabeth, I'm not familiar with this addon. Please check with the ExtendedForms team.

You should disable the auto close option if you have set it up using the ExtendedForms addon. If uninstalling the addon will automatically disable this option, you can also choose to uninstall it.
