Hi Shashank, Login to Neartail using the registered email account. Click on the form to open it. Edit page will be displayed. Click Preview. In the Preview page, theme settings will be displayed. Click Page. Enter the required text in the Business name (header / account name) and click Save. Please note that the theme settings are for your account and it will be applied for all the forms you create using your account. Neartail supports only public forms. You have enabled google forms features that require signin. You must disable these features in google forms > click on the addon icon > click Neartail > click More options > click Update or Customize to sync the changes and then use the Neartail share link to collect responses.
